Patient Registration

We welcome new patients who live within our practice area. All new patients will also be asked to complete a patient questionnaire allowing us to provide medical care until your medical notes are transferred to us – it can take some time for your records to arrive from a previous practice.

Registration

To register with us you will be asked for some identification, preferably a Driving Licence or Passport.  If you cannot provide ID you can be introduced by someone else who is currently registered at the practice.

We offer all new patients the opportunity to see our Practice Nurse or Healthcare Assistant for a new patient appointment – please arrange this via reception. If you have any long-term conditions such as high blood pressure, asthma, heart disease, COPD or diabetes, you will be invited within 6 months to see the Nurse or Healthcare Assistant for a review. Medical treatment is available from the date of registration.

If you move out of the practice area you will need to register with another practice immediately. We can provide information about how to find a new practice. Please contact reception for further information.

Please bring a completed registration form and new patient questionnaire with you when you come to register at the surgery. You may also wish to opt into receiving text messages and/or give consent for another individual to obtain information about your health.  Forms can be printed using the links below:
